School forms/sports forms

If your child will be participating in school sports for the 2023-2024 school year, please contact us soon to make his or her well care appointment. If your child has been seen for well care within the last 12 months, the form can be uploaded to our portal, emailed to charts@columbiapediatrics.com or faxed to our office at (562) 989-2911. We tend to book up quickly during the summer months, so please contact the office as soon as you are advised of the requirements of your particular school.
